New Patient Information
Yes, we are currently welcoming new patients to Cedar Health Centers.
New patients need to bring:
- Completed patient registration form
- Release of records form (if transferring from a different primary care provider)
- Signed HIPAA notice
- Current insurance card
- Valid photo ID

Returning Patient Information
Yes, we offer telehealth appointments for eligible visits. WGAT members also have 24/7 access to Doctor on Demand telemedicine services.
You will receive a confirmation call one day before your scheduled appointment. If you need to cancel your appointment, please contact our office as soon as possible. This helps us accommodate other patients who may need urgent care.

Cedar Health Centers exclusively serves Western Growers Assurance Trust (WGAT) and Pinnacle Claims Management Inc. (PCMI) insurance members.
